Hope Family Health Center is located within the Hope Academy, which provides an intimate setting that promotes integrated health to students and other children and adolescents in the community ages 5-21. Hope Family Health Center offers trauma-informed comprehensive pediatric services as well as screening for the social determinants of health and adverse childhood experiences. Hope Family Health Center maintains strong ties with the local community through a Community Advisory Council, composed of parents, school representatives, and local community leaders.

Preparing for a Visit
At Authority Health medical centers, we ask that you come to appointments with proper paperwork, including test results from other medical centers. It also helps if you are prepared to answer medical questions about existing conditions. When visiting an Authority Health medical center, please bring a valid driver’s license, proof of insurance (if applicable), prescriptions, and any questions you want to ask your health care provider. If your appointment is a follow-up appointment, it is helpful to bring prior test result records.
If you are a new patient, please arrive 30 minutes before your appointment to fill out paperwork. If you are a returning Authority Health patient, please arrive 15 minutes before your scheduled time.
During the COVID-19 Pandemic

- We are only allowing five patients in the lobby at one time.
- You may be asked to wait until a staff member calls you to enter the building.
- We will take your temperature (anyone with a temperature of 100 degrees or higher will be rescheduled).

What should I bring to my appointment?
When visiting an Authority Health medical center, please come prepared with a valid driver’s license, proof of insurance (if applicable), prescriptions, and any questions you plan to ask your healthcare provider. If your appointment is a follow-up appointment, please bring all test result records.
How early should I arrive to my appointment?
New patients should arrive to their appointments 30 minutes before their scheduled time. Patients who are already in the Authority Health system should arrive 15 minutes early.
What insurances does Authority Health accept?

What is my copay?
Your copayment (copay) is a flat fee that you pay for health care services, in addition to what your insurance covers. You can usually find the amount of your copay listed on your insurance card. You can also contact your insurance company to inquire about the amount of your copay. For example, some HMOs require a $10 copay for each office visit, regardless of the type or level of services provided during the visit.
What is my deductible?
Your deductible is determined by your insurance plan. It is a specified amount of money that you must pay before your insurance company will pay your claim. For example, some traditional insurance plans require you to have met a deductible before your services are paid. That deductible may be $500 for a single person and $1000 for a family. Any service can be applied against your deductible so that you can meet it.
I have Medicare; do I have a copay?
Medicare covers 80 percent of your visit, and you are responsible for 20 percent. The 20 percent of the visit you are responsible for is called coinsurance. A coinsurance is a type of insurance in which you pay a share of the payment made against the claim. If you have a secondary insurance, it usually covers the 20 percent you are responsible for.
