This link permits you to send messages regarding concerns or suggestions to the Authority Health Human Resources Department. All information is kept confidential and private by Authority Health to the extent allowed by law when submitting information through this link. Please explain your idea or concern as clearly as possible.  If you wish to be contacted directly regarding a suggestion, concern, or incident, please include your contact information.

This form generates a report that is normally reviewed during business hours and should not be used if you need an immediate response.

What to Do If You Have a Concern or Grievance at Authority Health

At Authority Health Centers (AHC), we are committed to making sure all patients are treated with respect and dignity. If you have a concern about your care or experience, we want to hear from you. We have a fair and efficient process to help resolve any issues that may arise during your care.

If you experience a problem that can be addressed quickly—such as a misunderstanding or minor issue—please speak with a staff member present at the time, such as a nurse, provider, or administrator. These are considered complaints, and we do our best to resolve them within 24 hours.

If your concern is more serious, cannot be addressed immediately, or involves a violation of your rights, it may be considered a grievance. Examples of grievances include: not receiving the care you expected, feeling your concerns weren’t shared with your provider, issues with informed consent or confidentiality, or experiencing any form of neglect, abuse, or unethical behavior.

To file a grievance, you can complete a Grievance Form, Authority Health Centers will provide,  send us an email, or speak to a staff member in person. We will help you document your concerns and ensure the issue is brought to the attention of our Chief Operating Officer (COO) and Risk Management Team. You will receive a response within 48 hours.

Once your grievance is received, it will be investigated thoroughly. We may ask you to share details such as the dates, times, individuals involved, and a description of the issue. You’ll be kept informed throughout the process, and we will work with you to find a resolution.

Qué hacer si tiene una inquietud o queja en Authority Health

En los Centros de Salud de Authority Health (AHC), estamos comprometidos a garantizar que todos los pacientes sean tratados con respeto y dignidad. Si tiene una inquietud sobre su atención o experiencia, queremos escucharla. Contamos con un proceso justo y eficiente para ayudar a resolver cualquier problema que pueda surgir durante su atención.

Si experimenta un problema que puede resolverse rápidamente—como un malentendido o un asunto menor—por favor hable con un miembro del personal presente en ese momento, como una enfermera, proveedor o administrador. Estos se consideran quejas, y hacemos todo lo posible por resolverlas dentro de las 24 horas.

Si su inquietud es más seria, no puede abordarse de inmediato, o implica una violación de sus derechos, puede considerarse una queja formal. Ejemplos de quejas incluyen: no recibir la atención que esperaba, sentir que sus inquietudes no fueron compartidas con su proveedor, problemas con el consentimiento informado o la confidencialidad, o experimentar cualquier forma de negligencia, abuso o comportamiento poco ético.

Para presentar una queja formal, puede completar un Formulario de Quejas que será proporcionado por los Centros de Salud de Authority Health, enviarnos un correo electrónico, o hablar en persona con un miembro del personal. Le ayudaremos a documentar sus inquietudes y aseguraremos que el asunto sea llevado a la atención de nuestra Directora de Operaciones (COO) y el Equipo de Gestión de Riesgos. Recibirá una respuesta dentro de las 48 horas.

Una vez que se reciba su queja, será investigada a fondo. Es posible que le pidamos compartir detalles como fechas, horarios, personas involucradas y una descripción del problema. Se le mantendrá informado durante todo el proceso, y trabajaremos con usted para encontrar una solución.
